Referee reports must be submitted by the Center Referee within 48 hours of the end of a match under the following circumstances:

If there is an incident in a game, a Supplementary Report must also be completed in addition to the Match Report. Report forms can be downloaded here.
An incident would include any of the following:
- Serious injury
- Send-off of a player, or ejection of a coach or spectator
- Termination or abandonment of game (e.g., game suspension or termination due to lightning)
- No show by a team
- Unusual field conditions
- Referee abuse or referee assault
- Other events out of the ordinary.
In the case of a send-off or ejection, for travel and Cup games, the player or coach’s pass must be included with the report.
