This is the updated information regarding the Coaches/Volunteers screening process for the 2016-2017 season.
NEW FOR THE 2016-17 SEASON: ALL PERSONS SUBMITTING A SCREENING APPLICATION MUST NOW REGISTER WITH USA HOCKEY PRIOR TO COMPLETING THE SCREENING APPLICATION. The USA Hockey participant number is now required on the Screening application. The link to USA Hockey Registration page is below. Select “Ice Players & Coaches” or “Ice Managers/Volunteers” as appropriate for your position – coach or volunteer board member, team manager, etc. There is no charge for “Volunteers” to register with USA Hockey, HOWEVER, “Volunteers” may not participate in any on ice activities.

ALSO NEW FOR 2016-17: The question on the screening application asking for the name of the CHC/USA Hockey program you are with has now been changed to a drop down list. Just select the program(s) name(s) from the list. You can select more than one. Hopefully this will clear up some confusion and eliminate some of the crazy answers we’ve had in the past.
For the 2016-17 season:
· Coaches & volunteers screened in 2015-16: do not have to be screened and should not submit a CHC online application. CHC Screening policy is to screen an individual coach/volunteer every 2 years.
· Coaches & volunteers personnel screened in any season prior to 2015-16 who were not rescreened in 2015-16 : MUST be rescreened, and submit a CHC online application

PLEASE NOTE: Student Coaches, age 17 and under, cannot be screened, nor should they submit applications on the above website.
When completing the application online, please double check your entries, especially your gender, date of birth, and social security number. Errors in these entries will cause delays in processing your application, and may result in your having to complete a new application.
